Description
1. A summary of your Profile; this section includes information about you and your personal situation. Provide details of usual schedule of eating habits, any health issues that affect food choices (only information that you feel comfortable sharing), food preferences, food preparation (cooking or eating out), etc. and any other pertinent information that affects your dietary habits.
2. Your food diary for the seven consecutive days of tracking; this can be the printed chart from MyFitness Pal or you can make your own charts with the food/beverages consumed for the seven consecutive days along with the specific dates.
3. Screen shots of your 20 charts (listed above)
Part II – Reflection on the following questions: (Include each question/statement with your response directly below each one.)
4. Overall, what did you discover about your diet and nutritional choices?
5. From which groups did you tend to eat more than is recommended?
6. Were there any groups for which you did not meet the recommendations?
7. What factors influenced your food choices during the seven days?
8. Identify at least five ways you can improve/maintain your diet. Be thoughtful and realistic!
The paper should be typed and a minimum of 4 pages (not including the screen shots) of content, 12-point font, double-spaced, with 1-inch margins. Be sure to spell-check, as spelling and grammar will count toward final grade! Save your final document as either a Word file or a pdf file. The Dietary Profile and Self Assessment paper must be submitted into the corresponding Assessment Dropbox in Folio no later than Sunday, Feb. 27 (11:59pm).
