Determining Community Needs and Services
Complete a windshield survey of your neighborhood or a nearby area. Do not go into areas that would be considered unsafe. The intent of the survey is to observe the social and environmental issues in the area as you drive through. You will be amazed at what you can see and the detail that you will observe. If possible, take someone along who can jot down notes. This will take about 30 min to 1 hour to complete sufficiently. When done write up a brief report of your findings with justification of the social and environmental determinants you have observed and how they could affect health either positively or negatively.

Activity 1: According to the CDC, health starts “in our homes, schools, workplaces, neighborhoods, and communities. We know that taking care of ourselves by eating well and staying active, not smoking, getting the recommended immunizations and screening tests, and seeing a doctor when we are sick”, (CDC, 2016)
In this exercise, you will review the social determinants of health as described by Healthy People 2020, the CDC’s planning framework for health improvement.
Look at the various factors described in the graphic below. Think about these from the point of view of where you live; which ones are more or less important.
Based on what you found in the windshield survey, discuss with your team each of the points.
Activity 2: Using the information discovered/interpreted during the class activity, create an infographic to share with political and community leaders about the more important social determinants of health in your home neighborhood.
Activity 3: Determining Needs and Resources
Based on the windshield surveys and other information gained begin to determine a plan to assess the needs and resources of the neighbor. If working in a team, choose one neighbor for the concentration of your plan. Choose a neighbor where there is a clear need for public health services.
Project Assignment:
Begin to develop a plan of action in the event of an outbreak or emergency (natural, bioterrorism, chemicals, radiation, etc) either from within the country or from a global source. What is the immediate response? Who will be involved in the coordination of the response? Who will be the first responders? Are they prepared, trained for different types of events? What will the communication structure be especially in the event that normal communication is affected negatively by the event?

Theory of Planned Behavior and Theory of Reasoned Action
Using the example above with Johnny, the Hawaiian boy. Select one of the behavioral theories or models explored and write a short summary (1 page) on how this model could be potentially used as a framework for helping patients manage diabetes better.

Discuss your review of the case and what happened. Share what model you chose to address the case and how the model would be carried out.

Download the Health Belief Model Case Study in the Important Document. Work with a team to complete the case study.
Activity 3: Analyzing a Community Problem
Together discuss what a community problem is, why it should be addressed, how you will collect information about the problem, and how to analyze the problem in order to prepare to develop a plan of action.
Project Assignment:
Continue to develop the plan of action in the event of an outbreak or emergency (natural, bioterrorism, chemicals, radiation, etc) either from within the country or from a global source. What about evacuation? Are there planned routes and procedures? Think about quarantining as opposed to evacuation. What if only are few are exposed? What about sheltering? What about caring for the injured or exposed? How will you provide for the normal healthcare needs of the community during the event? Your community assessment should have provided much of this information. You can use that as your starting point for developing the plan.
